
Gut Microbiome and Blood Sugar: The Link to Diabetes
People with type 2 diabetes are often told to focus on diet, weight, and genetics. That advice is important, but it can make the condition sound simpler than it really is. The usual explanation is that repeated rises in blood sugar and insulin place increasing pressure on the body. Over time, cells become less responsive to insulin, and the pancreas has to work harder to produce enough.
But that is only part of the picture.
Gut bacteria are now being studied as another factor in how the body controls blood sugar. The gut microbiome may influence inflammation, short-chain fatty acid production, appetite hormones, bile acid metabolism, and insulin sensitivity.
The science is still developing, but it could help explain why some people are more susceptible to type 2 diabetes than others. It may also point towards more personalised ways to support metabolic health in the future.
What Is the Gut Microbiome?
The gut microbiome is the community of bacteria, fungi, viruses, and other microbes living in the digestive tract, primarily the intestines. These microbes aid in digestion, breaking down certain fibres we can’t, producing metabolites, and ensuring harmful bacteria don’t overgrow.
They’ve got a secondary purpose, however.
Many of the metabolites they release help regulate the rest of the body. That could mean supplying the body with vital nutrients, or it could be downregulating inflammation or supporting insulin sensitivity.
What Does the Research Show About Gut Bacteria and Diabetes?
The research does not suggest there is one specific “diabetes bacteria”. Instead, the link seems to involve broader changes in bacterial balance, bacterial genes, and the substances gut microbes produce (e.g. the short-chain fatty acids (SCFAs)).
People with type 2 diabetes often have different gut bacteria patterns. Large microbiome studies have found differences in the gut bacteria of people with type 2 diabetes, including changes in bacteria linked with butyrate production and inflammation. However, the exact patterns vary between populations, so this is not a simple one-bacteria explanation.
Gut bacteria may affect blood sugar through the substances they produce. Research on high-fibre diets suggests that encouraging short-chain-fatty-acid-producing bacteria may support better HbA1c levels. Other studies have linked gut microbes with blood metabolites associated with insulin resistance.
The same food can affect people’s blood sugar differently. In one study of 800 people, researchers found large differences in how individuals responded to identical meals. Microbiome data helped predict those responses, suggesting gut bacteria may partly explain why universal diet advice does not work for everyone.
How Gut Bacteria May Affect Blood Sugar
There isn’t a single mechanism linking gut bacteria and blood sugar. Several different processes seem to increase the likelihood of type 2 diabetes developing.
For example, gut bacteria ferment fibre into SCFAs. The byproducts can help gut barrier function, reduce bodily inflammation, and decrease the chance of diabetes.
Similarly, other metabolites released by the bacteria can influence how glucose is metabolised or alter appetite. Each person may have a specific set of factors occurring. What’s clear is that whatever the mechanism, gut bacteria play an important role in blood sugar control.
Does Changing the Microbiome Improve Blood Sugar?
The real question is now what the underlying mechanism is. Can altering the microbiome help people with type 2 diabetes or pre-diabetes?
The answer appears to be yes.
In one notable study, a faecal transplant, which transfers the gut microbiome from one person to another, increased insulin sensitivity in individuals with metabolic syndrome.
Metformin is also a first-line medication in patients with type 2 diabetes. In mice, the drug was found to alter the microbiome to improve glucose tolerance. It may provide one potential explanation for why the drug is so effective.
Together, these findings could pave the way for new treatments. These could be specific probiotic formulas, a prebiotic regimen, or even new drugs that help support beneficial bacteria.
